Private Insurance (PKV):

    • Your health insurance number (Versicherungsnummer)
    • Your date of birth
    • The statement: "Hiermit widerspreche ich der Anlage einer elektronischen Patientenakte durch die [name of health insurance provider]"
    • Your signature, date, and location

For Private Insurance (PKV):Since the system is opt-in for private insurance, you simply need to not enroll if you do not wish to participate.
